Nurse Next Door Bayside is seeking a dedicated Care Co-ordinator RN Grade 4 in Elsternwick, VIC. This full‑time role coordinates aged care and disability services, serving as the central contact between clients, families and the care team to ensure seamless communication and continuity of care.

The ideal candidate has a nursing qualification or healthcare background, proven care coordination experience, and solid organisation, communication and EHR scheduling skills.
