ForHealth is seeking a Registered or Enrolled Nurse to join Southport Medical Centre to support GPs and patients through chronic condition management and preventative care. You will be part of a supportive team focused on collaboration and improving patient outcomes.

Responsibilities include health assessments, CCM coordination, and promoting preventative healthcare while growing the program and coordinating referrals within a multidisciplinary team.
