BlueCare is seeking a Senior Personal Carer to conduct initial client intake, assess needs, and develop personalised care plans. Role involves in office visits across Toowoomba North, Oakey & surrounding areas and coordination with the care team to review goals and outcomes.
You will travel to clients’ homes, provide self-care support and transportation assistance, and engage in community activities in line with the Care Plan. Requires Certificate III Aged Care and a valid driver’s license.
