About the group
Gratitude In Home Care is a registered aged care provider delivering Support at Home services built on transparent pricing, choice of worker and continuity of care. NorthPoint Disability is a registered NDIS provider delivering disability supports, including high-intensity supports, along with DVA community nursing. Both are part of one Preston-based group. We measure success by clients who stay, families who trust us and staff who want to keep working with us.
Purpose of the role
The Clinical Care and Compliance Lead keeps every client clinically protected and every client file compliant, across both brands. The role leads clinical assessment, care planning input and review, clinical risk and incident management, and clinical governance; owns client compliance against the Aged Care Act 2024, the Strengthened Aged Care Quality Standards and the NDIS Practice Standards; and builds the clinical capability of the workforce through training, competency assessment and education on matching clients to the right support.
The role sets the standard and audits against it. It does not roster,
allocate shifts or choose individual workers; it defines the clinical and matching criteria the Service Coordinator applies, and teaches the team to apply them well.
Key responsibilities
1. Clinical assessment, care planning establishment and review
- Lead the clinical assessment and review cycle for Support at Home, NDIS and DVA community nursing clients, including complex care, wound, continence, pain, falls, nutrition and hydration, cognition and dementia, behaviour, medication and palliative and end-of-life needs.
- Provide the clinical input to each care plan developed by the Client Care Partner so that clinical need, client goals, approved classification or NDIS plan and budget align; sign off the clinical elements of the plan.
- Provide direct nursing care and clinical education to clients, families and carers where required, including DVA Community Nursing clinical assessment and care plans within each episode of care.
- Monitor and evaluate clinical outcomes and deterioration; elevate and adjust care promptly; manage clinical risk for at-risk clients with documented mitigation and follow-up.
- Liaise with GPs, allied health, hospitals, dementia, continence, palliative and specialist services for integrated care; support hospital discharge, transition between providers and the Restorative Care and End-of-Life pathways.
- Support advance care planning, dignity of risk and supported decision-making conversations,
and document them.
2. Funding knowledge and budget utilisation
- Apply working knowledge of Support at Home (classification levels, quarterly budgets, care management funding, Restorative Care and End-of-Life pathways, Assistive Technology and Home Modifications scheme, co-contribution rules), NDIS plan types, support categories and Pricing Arrangements, and the DVA Community Nursing episode-of-care model.
- Establish recommendations based on unmet needs for client budget utilisation, and work with Client Care Partners and Service Coordinators to address under- and over-utilisation, unspent funds and service mix so every client receives the full benefit of their entitlement.
- Identify when a client's needs have changed and initiate reassessment, classification review or Support Plan review through My Aged Care, or a plan reassessment request with the NDIS.
- Provide the clinical justification for assistive technology, home modifications, nursing and allied health within the client's funding, and for NDIS high-intensity supports.
- Ensure services delivered are clinically justified, within the approved plan and documented to a claimable standard; return anything that is not to its owner before Finance claims it.
- Contribute clinical input to funding, pricing and compliance discussions with management.
3. Client compliance
- Own compliance of every client file: service agreement signed on or before the first service, informed consent, intake and clinical assessment, care plan, risk assessments, and documentation that meets program and claiming requirements.
- Run the pre-start compliance check on ever
