Barwon Health invites applications for a Clinical Pharmacist Grade 1.

This permanent part time role focuses on sub-acute services including rehabilitation, palliative care and geriatric pharmacy, providing clinical and operational support across McKellar Centre.

You will perform medication reconciliation, medicines management, patient counselling and discharge support within a multidisciplinary team.

Working closely with medical, nursing and pharmacy staff, you will contribute to education,
