MediRecruit is seeking an experienced Social Worker to join a Transition Care Program in Geelong, VIC. This part time role (0.6 FTE, 3 days per week) focuses on psychosocial assessment, discharge planning, and guiding families through aged care entry and support processes.

You will work within a multidisciplinary team to support older clients through transitions, with a focus on secure, sustainable post-hospital plans and adherence to relevant legislation.
