Kanda is seeking a Transitional Care Program Coordinator (RN) to guide clients from hospital to home, coordinating care plans and services. You’ll work with hospital teams, allied health specialists and funding bodies to ensure secure, effective recovery at home.

The role is office-based in Underdale with hybrid flexibility and a focus on seamless patient transitions, documentation and weekly reporting.
