IPC Health Ltd. is recruiting for a Part Time Aged Care Assessor to support the SAS program across Hobsons Bay, Brimbank, Melton and Wyndham.
The role focuses on holistic assessments, cooperative care planning and helping older clients live independently at home.
You will manage a caseload, use My Aged Care tools and maintain accurate client data, with travel across multiple LGAs and an option to work from home where feasible.
A fixed term until 30 June **** applies.
