You will perform client assessments, access respite services, and assist with service development and rostering for clients.
Work is in a multi-disciplinary team delivering responsive, client-centered care to diverse communities.
You will review procedures, reporting requirements, and collaborate with program stakeholders to grow the CHSP program and private client caseload.
