Alcohol and Other Drugs - Support Worker / Aboriginal Health Worker
Key Responsibilities
Assist clients to access health and social support services.
Undertake screening activities within scope of practice.
Coordinate referrals and follow-up activities.
Support care planning and case management.
Conduct outreach throughout Halls Creek and surrounds.
Advocate for clients and families.
Maintain accurate client records and case notes
Selection Criteria:
Experience working with Aboriginal communities.
Understanding of confidentiality and client-centred practice.
