Community Options Australia is seeking compassionate Aged Care Assessors to perform in-home assessments across the Southern Highlands, determining eligibility for supports under the My Aged Care program and helping clients remain independent at home and in their communities.
You will work autonomously within your region, reporting to the Assessment Team Lead, and collaborate with referral agencies to coordinate services.
