Sensible Care is seeking a Clinical Care Partner to provide clinical leadership within our Support at Home program in Victoria. You will oversee a caseload of participants living with chronic conditions, wounds, or palliative needs, making independent clinical judgments and shaping care plans to reflect each person's goals and culture.
In this autonomous role you will assess health risks, coordinate with GPs and specialists, and support Care Partners and other clinicians to deliver joined-up
