Palliative Care South East is seeking a Palliative Care Nurse Practitioner to provide autonomous, NP‐led care in home settings and advisory support to residential aged care facilities within our southeast Melbourne region.
You will lead comprehensive assessments, diagnose and plan care with the interdisciplinary team, while mentoring clinical staff and building capacity for a palliative approach across partner sites.
