Uniting AgeWell invites applications for a Care Advisor (Case Manager) in the West Metro Melbourne Home Care Program. You will manage your own caseload, championing clients’ independence and helping them stay in their own homes with supported services.

You will work from our Braybrook office with a supportive team, delivering a full time role from 9:00am to 5:00pm, Monday–Friday, contributing to outcomes and community impact.
