The Australian Childhood Foundation is seeking a Care Coordinator for the Out of Home Care program in Perth, WA. This permanent full time role focuses on coordinating tailored support for children and young people, working with carers, Therapeutic Specialists, and external agencies to deliver holistic care.

You will implement individual support plans and coordinate referrals, using an integrated case management approach to address risks and support needs in out-of-home care.
