With your person-led approach, you will:
- Meet prospective clients and families to understand their goals, strengths and needs.
- Develop clear care plans,



service agreements and budgets, and explain them in plain terms.
- Partner with our Client Services Team on Caregiver matching and help new clients settle in.
- Conduct regular reviews, escalate changing risks and coordinate hospital or residential care transitions.
- Monitor client budgets and keep documentation accurate and audit-ready.

What you’ll bring You are organised, calm under pressure and comfortable working on your own or as part of a team. You put each client’s rights and choices first, and you build trust quickly with clients, families and health professionals. To thrive in this role, you will have:

- Located in or near Gosford
- Experience managing Support at Home or Home Care Package clients.
- Experience working with older people in a care or health setting.
- Solid organisation, time management and communication skills.
- A current Australian driver’s licence.
- Valid Australian working rights and willingness to complete a National Police Check.

It’s not essential, but it would be wonderful if you also have:
- A tertiary qualification in nursing, social work, allied health or a related field.
- Knowledge of local health and community services.
- Familiarity with the Aged Care Act 2024 and the Strengthened Aged Care Quality Standards.
