Murrumbidgee Local Health District in Wagga Wagga welcomes an experienced Dietitian to join the Transitional Aged Care Program. In this role you will assess nutrition needs, develop individualized care plans and deliver targeted interventions to support recovery and independence for older adults returning home from hospital. You’ll work within a multidisciplinary team, manage a caseload across up to 25 TACP packages (subject to hours) and conduct home visits to coordinate care with clients,
