We are looking for two people to provide short-term, full-time Case Manager cover , based in Baulkham Hills , during the following periods:

- 9 November 2026 to 30 November 2026

- 16 November 2026 to 1 December 2026

With Lifestyle Solutions, you'll play a vital role in supporting children, young people, families, and carers through strength-based, person-centred case management. You'll work collaboratively to deliver meaningful outcomes by developing tailored support plans, coordinating services, and advocating for the young people in your care.

- Two short-term, full time cover positions – SCHADS Level 5 – $53.43 per hour plus super

- Based at our Baulkham Hills office with travel to carers' homes

What You'll Bring

- Demonstrated experience in case planning for children and young people, preferably within Foster Care or Out-of-Home Care

- Knowledge of the Children and Young Persons (Care and Protection) Act 1998

- Experience supporting children and young people who have experienced trauma, disability, or complex behaviours

- Robust analytical, assessment, and problem-solving skills

- Excellent written communication and record-keeping abilities





- Ability to work flexibly and collaboratively with a range of stakeholders

What You'll Be Doing

- Manage a caseload of children and young people, working closely with their families and carers to achieve positive outcomes

- Complete comprehensive assessments and develop, implement, and review individual case plans

- Coordinate intervention strategies, including risk assessments, referrals, and program applications

- Prepare reports, legal documentation, and support child protection-related court processes where required

- Facilitate access to services and monitor their effectiveness in meeting individual needs

- Advocate for children and young people by ensuring their voices, goals, and wellbeing remain at the centre of care planning
