About the Position

Hiba HealthCare is seeking a qualified and experienced Community Worker to join its team in Morwell, VIC 3840.

The Community Worker will assess clients' needs, develop individualised support plans, coordinate community services and assist individuals in improving their independence and community participation.

Key Responsibilities

- Assess clients' social, practical and personal needs and develop appropriate support plans.

- Assist clients in accessing community services, healthcare, housing and other relevant support programs.

- Provide information, advocacy and practical assistance to individuals experiencing disadvantage or social difficulties.

- Coordinate referrals and liaise with community organisations, government agencies and service providers.

- Support clients in developing independent living skills and improving community participation.

- Identify clients requiring urgent assistance and arrange appropriate support and referrals.

- Plan and coordinate community outreach activities and support programs.

- Work with clients,



families and healthcare professionals to review and update individual support plans.

- Monitor client progress and maintain accurate case notes, assessments and reports.

- Identify community needs and assist in improving access to available support services.

- Build positive relationships with community organisations and external stakeholders.

- Assist in developing and reviewing community programs and service procedures.

- Promote clients' rights, dignity, independence and cultural safety.

- Ensure compliance with organisational policies, safeguarding requirements and workplace health and safety standards.

Qualifications and Experience

The successful applicant must have:

- A relevant Diploma-level qualification or higher in Community Services, Social Services, Welfare or a related field.

- At least two years of relevant experience in a Community Worker or similar role.

- Experience assessing client needs and developing individualised support plans.

- Knowledge of disability, aged-care and community support services.

- Solid communication, interpersonal and documentation skills.

- Experience working with community organisations and external service providers.

- Ability to work independently and as part of a team.

- A valid Australian driver's licence.

- Relevant police and worker screening checks, where applicable.

- Current Australian work rights.
