Mersey Care NHS Foundation Trust is seeking a Band 5 physical health nurse to join district nursing teams providing home-based care across an 8am–8pm, 7 days a week model. You will work within a multi-disciplinary team to deliver safe, effective care in patients' homes, and you will be supported to develop skills through mentoring and structured supervision.

The role requires a registered nurse with district nursing experience, robust clinical assessment skills and the ability to work
