The Royal Melbourne Hospital is seeking a part time Clinical Nurse Consultant Coordinator for the Older Person Care Hub in Melbourne. This role focuses on leading triage, service coordination and timely assessments to help older adults remain safely at home and avoid unnecessary hospital visits.
As part of the team, you will work with geriatricians and allied health staff, oversee the four-week care pathway and support expanded seven-day operations from 2027, including occasional weekend shifts.
