Alcohol and Other Drugs - Support Worker / Aboriginal Health Worker
Key Responsibilities
- Assist clients to access health and social support services.
- Undertake screening activities within scope of practice.
- Coordinate referrals and follow-up activities.
- Support care planning and case management.
- Conduct outreach throughout Halls Creek and surrounds.
- Advocate for clients and families.
- Maintain accurate client records and case notes
Selection Criteria:
- Experience working with Aboriginal communities.
- Understanding of confidentiality and client-centred practice.
