Sydney Local Health District is seeking a Social Worker Level 3 for the Out of Home Care Health Pathway Program based at Croydon Health Centre. The role involves coordinating multidisciplinary health reviews and developing Health Management Plans to address the health and wellbeing needs of children in OOHC.

The ideal candidate will hold a Social Work degree with AASW eligibility, possess trauma-informed assessment expertise, NSW Driver's Licence, and be able to work three days per week
