Are you an experienced Disability Support Worker who combines practical care skills with patience, empathy, and a genuine commitment to making a difference?

Chosen Family is recruiting dedicated Support Workers for a new Supported Independent Living (SIL) opportunity in Rosemeadow, NSW!

We’re looking for capable, compassionate professionals with experience in manual handling, wheelchair support, and stroke-related disability support. If you understand the importance of safe transfers, effective communication, and preserving a person’s dignity and independence, we’d love to hear from you.

This is your chance to put your skills to meaningful use, build a positive working relationship with a participant, and contribute to a consistent, person-centred support team.

About the Opportunity

You’ll be supporting a participant transitioning from hospital into a fully accessible home in Rosemeadow. Your role will focus on providing safe, respectful assistance with daily living, mobility, communication, and wellbeing in line with the participant’s individual support plan.

We’re building a reliable team of Support Workers who understand that quality support means recognising the person behind the support needs.

What You’ll Do

As part of the support team, you will:

- Assist with safe pivot transfers and the use of walking belts,

following the participant’s support plan and approved manual handling procedures.
- Support wheelchair mobility, positioning, and safe movement as required.
- Provide assistance with daily living activities while promoting dignity, choice, and independence.
- Communicate patiently and adapt your approach when supporting a person with aphasia.
- Monitor wellbeing and promptly communicate any concerns or changes in support needs.
- Maintain accurate shift notes and documentation to support continuity of care and compliance.
- Collaborate with colleagues to provide consistent, safe, and respectful support.

What We’re Looking For

Relevant hands‑on experience is important for this role.

We’re seeking applicants who have:

- Practical experience with manual handling, particularly pivot transfers and walking belt use.
- Experience supporting wheelchair users.
- Experience supporting people following a stroke or living with stroke-related disabilities.
- Clear spoken and written English communication skills.
- Experience supporting people with aphasia, or the demonstrated ability and willingness to learn appropriate communication strategies.
- A patient, observant, respectful, and person-centred approach.

- The ability to follow individual support plans, safety procedures, and workplace policies.

You’ll Stand Out If You…

- Can confidently demonstrate protected mobility and transfer techniques.
- Understand how stroke-related disabilities can affect communication and everyday functioning.
- Know how to adjust your communication style to support a person’s understanding, expression, and choice.
- Are dependable, proactive, and comfortable working collaboratively within a support team.
- Take pride in skilled conduct, accurate documentation, and delivering consistent care.
