Western Sydney Local Health District is seeking an Admin Officer Level 3 to join the ACILS at Westmead Hospital. You will coordinate residential aged care placements, support discharge planning and liaise with hospitals, aged care providers and social workers to improve patient flow.

In a collaborative setting, you will build relationships with facilities and stakeholders, manage records, provide timely information and help patients and families navigate aged care options, with opportunities
