Communify is seeking a Case Manager to support families experiencing risk of homelessness or tenancy loss. You will provide intake, assessment, casework and referrals to help families access information, advice and advocacy.

Working with the Sustaining Tenancies & Community Care Team, you will develop case plans, coordinate supports and strive for long‑term secure housing outcomes while delivering compassionate, person‑centred service.
