At Home Together is seeking a compassionate Aged Care Coordinator / Care Partner in Canberra. This permanent, full-time role prefers Enrolled Nurses (EN) or Registered Nurses (RN) with community aged care experience.

You will manage a portfolio of clients, coordinate care plans, onboarding and regular reviews, and work with families and healthcare professionals to deliver secure, person-centered in-home services across Canberra and surrounding regions.
