About Our Client
Our client is a growing, well regarded WA disability provider. They’re expanding their NDIS team and want an intake specialist to own how new participants come on board.

The Role
Every new participant starts with you. From first contact through assessment to service start, you’ll guide each referral and make sure their needs are understood and the right supports are ready on day one. Once everything is in place, the handover to the operations team comes with a clear, complete picture.

Based in Mirrabooka, the role includes travel to participant homes, hospitals and service sites.

What You’ll Do

- Manage referrals and the complete intake and onboarding process
- Review NDIS plans, funding, goals and support needs
- Coordinate assessments, meet-and-greets and risk assessments
- Flag complex needs and clinical risks early
- Prepare service agreements, consents and onboarding paperwork
- Line up staffing, training and resources before services begin
- Build strong links with families, Support Coordinators and hospitals

What You’ll Bring





- Bachelor of Nursing with current AHPRA registration
- 2+ years’ relevant professional experience
- Direct, hands-on NDIS participant intake experience. This is essential and non-negotiable.
- Solid knowledge of NDIS plans, funding and service agreements
- Understanding of risk assessment, complex needs and safeguarding
- Strong organisation and the confidence to work autonomously

Please note: aged care or general nursing experience alone, without NDIS intake, won’t meet the brief.
