Guardian Nursing Agency (GNA Services) is seeking a compassionate, reliable and dedicated Aged Care Support Worker to provide quality, person-centred support to older Australians in their homes and communities.

About the Role

As an Aged Care Support Worker, you will assist clients with their daily living activities and provide practical and emotional support in accordance with their individual care plans.

You will play an important role in helping clients remain safe, comfortable and independent in their own homes.

Key Responsibilities

- Provide personal care assistance, including showering, grooming and dressing.
- Assist clients with mobility, transfers and the use of mobility aids, where required.
- Support clients with toileting and personal hygiene.
- Provide medication prompting or assistance only as authorised, trained and required under the client's care plan.
- Assist with meal preparation and light domestic duties, where required.
- Provide companionship and emotional support.
- Support clients to attend appointments and participate in community activities, where required.
- Monitor and report changes in clients' health, wellbeing or support needs.
- Maintain accurate progress notes and service records.
- Follow individual care plans, workplace policies and relevant safety procedures.
- Respect clients' privacy, dignity, cultural preferences and right to make their own decisions.

Essential Requirements

- Experience in aged care, personal care or community support.
- A current National Police Clearance.
- Current First Aid and CPR certification,



where required for the role.
- A valid Australian driver's licence, where driving is required.
- Access to a reliable and appropriately insured vehicle, where travel between clients is required.
- Good communication and interpersonal skills.
- Ability to work independently and follow care plans and instructions.
- A compassionate, patient, respectful and professional approach.
- Commitment to maintaining confidentiality and safeguarding older people.

Desirable Qualifications

- Certificate III in Individual Support (Ageing or equivalent).
- Relevant aged care, home care or personal care training.
- Manual handling training and experience.
- Medication assistance training, where applicable.
- Experience supporting older people with mobility limitations or additional support needs.
