About the Role

We are seeking a motivated and compassionate Physiotherapist to join our community-based team, providing high-quality physiotherapy services to clients in Beaudesert and surrounding areas.

This role involves delivering individualised physiotherapy assessment and treatment within clients' homes and community settings, supporting people to maintain or improve their mobility, strength, balance, independence and overall quality of life.

The successful candidate will enjoy working autonomously while collaborating with clients, families, carers and other healthcare professionals to deliver person-centred care.

Key Responsibilities

- Conduct comprehensive physiotherapy assessments within clients' homes and community settings.
- Develop and implement individualised treatment plans based on clients' needs, goals and functional abilities.
- Provide interventions targeting mobility, balance, strength, falls prevention, pain management and functional independence.
- Support clients with rehabilitation, mobility equipment prescription and recommendations, and safe transfers.
- Deliver home exercise programs and provide education to clients, families and carers.
- Complete clinical documentation, progress reports, treatment plans and other required records accurately and promptly.
- Monitor client progress and modify treatment programs as required.
- Communicate and collaborate with GPs, occupational therapists, nurses, support coordinators and other relevant stakeholders.
- Identify and manage clinical risks, including falls risk and home safety concerns.
- Manage a community-based caseload, including appointment scheduling, travel and ongoing client follow-up.
- Maintain compliance with professional standards, workplace policies and relevant legislation.

About You

To be successful in this role, you will have:

- A Bachelor of Physiotherapy or equivalent recognised qualification.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).




- Current driver's licence and access to a reliable vehicle for community visits.
- Strong clinical reasoning, assessment and treatment planning skills.
- Excellent communication, interpersonal and organisational skills.
- The ability to work independently while contributing positively to a multidisciplinary team.
- A compassionate, flexible and client-centred approach to care.
- Confidence managing a varied caseload and prioritising competing demands.

Desirable Experience

- Experience in community physiotherapy, home-based rehabilitation or aged care.
- Knowledge of falls prevention, balance retraining, functional mobility and strength programs.
- Experience working with older adults and people living with chronic or complex health conditions.
- Familiarity with mobility aids, walking frames and functional equipment recommendations.
- Experience working with Home Care Packages, Support at Home, NDIS or other community funding programs.

Work Location

This position will service Beaudesert and surrounding communities. Travel between client homes and community locations is an essential component of the role.
