Flourishing Lives Disability Services

Maitland

Initially Casual / Ad Hoc | Pathway to Permanent Position | Flexible Community-Based Role

About the opportunity

Flourishing Lives Disability Services is seeking an experienced, practical and person-centred Registered Nurse to join our growing clinical services team. This is a varied community-based role combining Community Nursing, participant clinical assessment and care-plan development, clinical guidance and supervision, and support for Disability Support Workers delivering complex and High Intensity supports.

The position will initially be offered on an ad hoc/casual basis, with hours based on participant clinical needs, Community Nursing referrals, care-plan development and clinical support requirements.

As FLDS continues to grow its Community Nursing and High Intensity Support services, our intention is for the role to develop into an ongoing permanent position with regular hours.

This creates an opportunity for the successful RN to join us during the development of our clinical services and play an important role in shaping FLDS's clinical practice and governance framework.

About the role

As our Registered Nurse, you will work collaboratively with participants, families and support networks, Disability Support Workers, FLDS management and participants' treating health practitioners.

The position combines direct nursing practice with a broader clinical support function.

Our current Module 1 - High Intensity Daily Personal Activities Module provides for clinical assessment, development and review of participant care plans, incorporation of treating-practitioner instructions, clinical guidance and supervision, participant/family education and escalation of clinical concerns.

Community Nursing

You will:

- Deliver person-centred Community Nursing supports to NDIS participants in their homes and community.
- Undertake nursing assessments, monitoring and clinical interventions within your professional scope and demonstrated competence.
- Provide nursing supports such as medication management, wound care, diabetes management and other participant-specific nursing interventions appropriate to your scope.
- Monitor participant health and identify changes, deterioration or emerging clinical risks.
- Coordinate appropriate escalation, referral or review where clinical concerns arise.
- Liaise with participants' GPs, specialists, pharmacists, allied health professionals and other treating practitioners as required.
- Maintain accurate and timely clinical records.

Clinical assessments and care-plan development

A key part of this position will be helping ensure participants with complex or High Intensity support needs have clear, individualised clinical guidance for the staff supporting them.

You will:

- Undertake clinical assessments for participants requiring nursing or High Intensity Daily Personal Activities.
- Develop and review participant-specific clinical care plans within your professional scope.
- Work collaboratively with participants and their chosen support networks when developing clinical supports.
- Ensure care plans clearly identify clinical instructions, monitoring requirements, known risks, escalation pathways and emergency responses.
- Incorporate relevant directions and clinical information from GPs, specialists and other treating health practitioners.
- Review plans when participant needs change and at appropriate scheduled review points.
- Identify circumstances requiring referral back to the participant's treating practitioner or another appropriately qualified health practitioner.

FLDS's current Module 1 framework provides for initial assessment and care-plan development, review after supports commence and further review when a participant's health or support requirements change.

Clinical supervision and Disability Support Worker development

You will also provide clinical support to Disability Support Workers who assist participants with complex health-related supports.

Depending on the participant and clinical activity, this may include:

- Providing participant-specific clinical guidance and instruction.
- Supporting supervised workplace practice.
- Observing workers performing relevant clinical support tasks.
- Providing feedback and identifying areas requiring additional practice.
- Assessing demonstrated clinical skills within your professional scope and FLDS's clinical governance framework.
- Where appointed as the appropriate Clinical Reviewer, making documented clinical competency and participant-specific authorisation decisions in accordance with FLDS procedures.
- Identifying skill gaps and recommending additional supervised practice, refresher education or reassessment.
- Providing practical clinical guidance while maintaining clear professional boundaries and appropriate escalation arrangements.

FLDS's Clinical Training, Competency, Delegation and Authorisation Procedure distinguishes training, supervised practice, competency assessment and participant-specific authorisation rather than treating completion of training as automatic evidence of competence. For example, FLDS's Training Plan for Subcutaneous Injections provides for theory, simulation, workplace-based assessment and participant-specific training before independent support, with final competency determination and authorisation by an RN or appropriately qualified health practitioner.

Clinical governance

You will work collaboratively with FLDS management to support safe, practical and proportionate clinical governance across our services.

This may include:

- Reviewing relevant participant clinical documentation.
- Supporting medication safety and medication-management practices.
- Supporting infection prevention and control.
- Providing clinical input following incidents or changes in participant health.
- Supporting appropriate clinical escalation pathways.
- Contributing to the review and continuous improvement of clinical policies, procedures and documentation.
- Supporting safe implementation of High Intensity Daily Personal Activities.
- Liaising with external treating practitioners and clinical providers where appropriate.

About youWe are looking for someone who combines sound nursing judgement with a practical and collaborative approach.

You will be comfortable working autonomously in the community while also enjoying the opportunity to support a broader disability services team.

Essential requirements

Why join FLDS?

This is a little different from a traditional Community Nursing position.

You'll have the opportunity to combine hands-on nursing with clinical leadership and service development, while maintaining a strong focus on participants and their individual goals.

Start flexibly and grow with us

The position will initially operate on an ad hoc/casual basis, responding to Community Nursing referrals, participant assessments, care-plan reviews and clinical support requirements.

We are actively developing this area of FLDS and expect the nursing workload to grow alongside our Community Nursing and High Intensity Support services.

Our intention is for the position to transition into an ongoing permanent role as the clinical caseload develops.

For the right RN, this provides an opportunity to join us early and help shape how our clinical services develop.

You'll have the prospect to:

- Work autonomously in the community.
- Develop meaningful ongoing relationships with participants.
- Combine direct nursing practice with broader clinical responsibilities.
- Help Disability Support Workers develop confidence in safely supporting people with complex needs.
- Contribute to the development of FLDS's clinical systems and governance.
- Work collaboratively with participants and their treating practitioners.
- Be part of an organisation focused on developing sustainable, high-quality disability supports.
- Potentially grow your hours and responsibilities as our Community Nursing and clinical services expand.

About Flourishing Lives Disability Services

Flourishing Lives Disability Services supports people with disability to live safely, independently and meaningfully in their homes and communities.
