Help deliver excellent support and develop the people providing it
Fortified Support is seeking an experienced Senior Team Leader / Support Worker who enjoys providing hands-on support, teaching others and building positive relationships with participants and families.
Based at Community Collective in Mount Pleasant, you will combine rostered direct support with dedicated time for worker coaching, participant visits and service follow-up across the community.
This is an opportunity for someone with strong disability and aged care experience who can lead by example, communicate clearly and help workers build confidence and maintain high standards.
About the role
Reporting to the Operations Director, you will:
- Provide respectful, person-centred support during rostered shifts at Community Collective.
- Coach and mentor workers, demonstrate good practice and reinforce participant support plans and preferences.
- Help new workers settle into their roles through introductions, practical guidance and follow-up.
- Observe staff practice and provide factual, constructive notes to management to support performance reviews.
- Visit new and existing participants to assist with onboarding, understand their needs and check how services are going.
- Support participant and worker introductions and communicate relevant information to the scheduling team.
- Review case notes and handovers for clarity and completeness, following up gaps with workers.
- Identify concerns, changes in support needs and opportunities to improve services, escalating these promptly.
- Work closely with management, scheduling, compliance and our Registered Nurse.
Management retains responsibility for formal performance management, and clinical oversight remains with our Registered Nurse. Teaching and coaching will be within your demonstrated competence and authorised responsibilities.
How the role will work
We envisage approximately three days of direct support and two days of leadership and participant work each week, with the final arrangement agreed around service needs.
Dedicated time will be allocated for community visits, coaching, documentation and follow-up. Full-time availability is preferred, although we will consider four days per week for the right person.
About you
You will bring:
- Demonstrated hands-on experience in disability support and aged care, with a sound understanding of the NDIS.
- Experience leading, mentoring or coaching support workers.
- The ability to explain tasks clearly, check understanding and give respectful, constructive feedback.
- Strong communication skills and confidence working with participants, families and support networks.
- Clear, factual documentation skills and confidence using digital rostering and participant record systems.
- Good organisation, sound judgement and the ability to follow through on agreed actions.
- A commitment to participant choice, dignity, privacy and independence.
- A willingness to provide direct support as an integral part of your role.
A relevant qualification in disability, individual support, community services, nursing or a related field will be highly regarded.
You will also need a current driver’s licence, access to a suitable insured vehicle for community visits, relevant worker screening clearances and current First Aid and CPR certificates. A Working with Children Check will be required if your duties involve children.
What we offer
- A varied role combining direct support, worker development and participant engagement.
- Dedicated time to carry out leadership responsibilities.
- Support from approachable directors and an established office team.
- RN guidance for clinical matters.
- The opportunity to contribute to how our services develop and improve.
