South Western Sydney Local Health District is seeking an Enrolled Nurse to join the Transitional Aged Care Program’s residential-based team. The role focuses on reablement for older adults, with assessments, goal setting and discharge planning conducted in collaboration with the MDT and in partnership with families.
You’ll work under the supervision of a registered nurse, delivering person-centred care and contributing to better health outcomes in a supportive, multidisciplinary workplace.
