IPC Health is seeking a Part Time Aged Care Assessor to deliver Home Support Aged Care assessments across Hobsons Bay, Brimbank, Melton and Wyndham. You will conduct holistic assessments for clients 65+ (50+ for Aboriginal or Torres Strait Islander people) using the Integrated Assessment Tool via My Aged Care.
You will develop cooperative Support Plans to promote independence, empower clients, and ensure data accuracy, quality care and regulatory compliance while contributing to service
