St Vincent's Care at Home in Toowoomba is seeking a Registered Nurse to deliver acute-equivalent care in the community. You will work with MCAH leads, VMOs and multidisciplinary teams to assess needs, develop care plans and coordinate protected transitions from hospital to home.
You will provide IV therapies, wound care and patient education, contributing to reduced admissions and improved outcomes in a patient-centred model of care.
