Many of the older persons living at Prague House have a history of homelessness or have been at risk of being homeless.
Prague House offers a person-centred model of care that supports older persons to achieve their individual goals and strengthen their wellbeing.
Located in Kew, Prague House is vital part of St Vincent's Hospital Melbourne Continuing Care Services, a key clinical service program of SVHM and broader SVHA Public Hospital division.
Prague House provides a safe, welcoming and therapeutic setting with the multidisciplinary team working collaboratively to support the older person, promoting independence, dignity and community.
About the Role As aRegistered Nurse at Prague House, you will be an integral member of a multidisciplinary team providing high-quality nursing care in a unique homelike setting.
You will use your clinical expertise to support older persons with complex physical, cognitive and mental health needs while delivering person-centred care that promotes independence and quality of life.




You will provide supervision to Enrolled Nurse and Health Care Worker staff to ensure the provision of high quality care.
This role provides an excellent opportunity to develop your expertise in aged care, mental health and complex care nursing while working alongside experienced clinicians and an engaged multidisciplinary team.
You will contribute to creating a positive therapeutic environment that supports recovery, resilience and meaningful community participation.
Classification Registered Nurse Grade 2 (YP3-YP9) About You We are seeking a motivated and experienced Registered Nurse who is passionate about providing exceptional care to older persons with complex mental health and care needs.
You will demonstrate a commitment to person-centred practice and work collaboratively with residents, families, carers and the multidisciplinary team to achieve positive outcomes.
