ABOUT THE ROLE

Mounties Care Home care is searching for a dedicated Client Intake Consultant to join our team on a full-time at our office in Mount Pritchard, NSW!

The successful candidate will be the first point of contact for people enquiring about our home care services.

More specifically, some of your key duties will include but are not limited to:

- Respond promptly to inbound enquiries and follow up current leads and referrals by phone.
- Meet prospective clients and families at home, in the office and at community events.
- Assess needs, eligibility and funding, and explain services and costs clearly.
- Guide clients through My Aged Care,



service agreements and handover to Care Managers.
- Record every lead in the CRM and report weekly on enquiries and conversions.
- Build relationships with hospitals, GPs, allied health and community referrers.
- Meet aged care quality, privacy and conduct obligations, and escalate risks or complaints.

ABOUT YOU

The successful candidate will ideally have the following:

- 2+ years in intake, sales, customer service or care coordination.
- Excellent phone and face-to-face communication, with genuine empathy for older people.
- Strong organisation, follow-up and CRM skills.
- National Police Check, NDIS works check, WWC, driver's licence and reliable vehicle.
- Aged care, home care or disability experience.
- Knowledge of Support at Home and My Aged Care.
- Certificate III/IV in Individual Support or Ageing Support. (desirable).
