Looking for a nursing role with real variety, where no two days are the same?
At Burnie Brae, that’s what you’ll get.
We’re looking for an experienced and compassionate Registered Nurse to join our Support at Home team. This role has three parts. You’ll deliver hands-on community nursing care, step in as a Care Partner during leave to keep client care continuous, and coordinate our nursing team’s scheduling, escalations and day-to-day workflow.
Based at our Chermside office, you’ll support older people in their own homes so they can stay safe, independent and connected in the place they most want to be. If you’re a confident clinician who also enjoys keeping a team running smoothly, this role is for you.
At Burnie Brae, we know great care starts with great people. That’s why we’ve created an environment where our team feels supported, valued and empowered to do their best work every day.
As part of our Support at Home team, you’ll enjoy:
- A varied role that blends clinical care, care coordination and nursing leadership
- Diverse and rewarding work across restorative care, chronic disease management and end-of-life care
- The chance to build trusted relationships with clients, families and a collaborative multidisciplinary team
- Salary packaging of up to $15,900, plus an additional $2,650 for meals and entertainment
- 17.5% annual leave loading
- Access to our Employee Assistance Program (EAP)
- Staff discounts across Burnie Brae services, health insurance and travel
- Discounted gym memberships, physiotherapy and massage services
- Blue Light Card savings on retail and everyday expenses
- Recognition and reward programs that celebrate your contribution
- The opportunity to help shape the future of community aged care through Australia’s new Support at Home program
About the role
As our Registered Nurse, you’ll:
- Deliver community-based clinical care and develop nursing care plans for Support at Home clients
- Undertake comprehensive clinical assessments, including falls risk, pain, skin integrity, nutrition, continence, cognition and mental health
- Safely administer and manage medications, and recognise and respond to clinical deterioration
- Support clients with restorative care, advance care planning and end-of-life care
- Step in as backfill Care Partner during planned and unplanned leave, so clients keep continuity of care
- Act as the first point of contact for nursing team escalations, triaging clinical and operational issues
- Coordinate nursing team scheduling, including visits, assessments, reviews and leave cover
- Provide clinical leadership, delegation and support to Support Workers
- Monitor client budgets against Support at Home funding and escalate variances
- Maintain accurate documentation and ensure compliance with Aged Care Act 2024, the Aged Care Quality Standards and the Code of Conduct for Aged Care
- Identify and report incidents, including SIRS reportable incidents, and contribute to clinical audits and quality improvement
About you
You’re a skilled Registered Nurse who is just as comfortable coordinating a team as delivering hands-on care.
You’ll also bring:
- A Bachelor of Nursing and current AHPRA registration as a Registered Nurse, with no conditions that limit practice in this role
- At least two years’ post-registration clinical experience
- Current First Aid and CPR certificate
- Current Australian driver licence and access to a reliable, roadworthy vehicle for home visits
- Current National Police Check (or willingness to obtain) and any required aged care worker screening
- Strong organisation and time management skills, and the ability to switch between clinical, coordination and admin work
- Sound clinical judgement and confidence working autonomously in client homes
- Excellent written and verbal communication skills
- A genuine commitment to person-centred, rights-based and culturally safe care
Experience in community or home care, palliative care, wound management or chronic disease management will be highly regarded.
Experience in care coordination or case management within Support at Home or Home Care Packages, or coordinating schedules and clinical escalations, will also be highly regarded.
For more than 40 years, Burnie Brae has supported older Australians to live healthy, connected and independent lives.
As an award-winning not-for-profit organisation, we’re proud of our supportive, people-first culture and the positive impact we make across our community.
As Australia transitions to the new Support at Home program, we’re excited to continue delivering creative, person-centred care, and we’d love you to be part of that journey.
