Community Care Manager

I’m currently partnering with a well-regarded not-for-profit aged care provider to recruit a Care Manager for their home care team on a 12-month contract.

Main Duties

- Lead client assessments and develop person-centred care plans
- Coordinate services for older people under the Support at Home program




- Manage a wide-ranging caseload, from domestic and allied health support through to complex case management
- Monitor client outcomes and review care as needs change
- Build strong relationships with clients, families and service providers
- Ensure accurate records and compliance with program requirements
- Support the team through an on-call roster

Essentials

- Experience in aged care case management or care coordination
- Diploma of Community Services (Case Management), Nursing, Aged Care or related qualification
- Working knowledge of Support at Home and CHSP
- Strong communication, organisational and admin skills
