The Role

We are seeking an experienced Care Partner Team Lead – Support at Home, you’ll be the link between clients, families, and support workers. Ensuring care is delivered with empathy, accuracy, and heart. This is a dynamic role where you’ll combine client engagement, care coordination, and rostering responsibilities, making every day varied and rewarding.

Key Responsibilities

- Leading and coordinating day-to-day activities and priorities.
- Providing guidance, support and direction to team members.
- Monitoring performance, progress and deliverables against agreed objectives.
- Identifying opportunities for process improvement and implementing best practices.
- Supporting stakeholder engagement and escalating complex issues where required.
- Promoting accountability,



collaboration and continuous improvement across the team.

About You

- Experience in care coordination, rostering, or client services.
- Knowledge of the NDIS and/or Aged Care sector (especially self-managed NDIS & HCP).
- Excellent communication and time management skills.
- A proactive, can-do attitude with a passion for client care.
- Confident computer skills (rostering systems or CRMs a bonus).
- Police Check, WWCC, NDIS Worker Screening Check (or willingness to obtain).
- Current First Aid & CPR (or willingness to obtain).
- Minimum of two doses of COVID-19 vaccination.
- Comfortable in quick-paced, regulated, or changing environments
- Proactive and committed to continuous improvement
