Sydney Children’s Hospitals Network (SCHN) is seeking a Priority Populations Care Navigator for the PEACH program. This temporary part time role is based at Westmead and aims to develop sustainable pathways to improve access to health services for priority populations using innovative technology.

You will coordinate project activities, monitor KPIs, and collaborate with Local Health Districts and SCHN teams to embed SCHN-wide systems and navigation pathways that enhance access and reduce
