The Transitional Aged Care Program based at War Memorial Hospital provides time limited, multidisciplinary support and low level rehabilitation and reablement therapy to community clients.
The AIN / AHA will work within a energetic multidisciplinary team to provide therapeutic intervention and support to aged clients in their homes following a period of hospital care, with a focus on maximizing independence in daily activities.
