Austin Health in Melbourne is seeking a Heart Failure Nurse Practitioner or Candidate to optimise patient outcomes across ED, inpatient, and outpatient settings, including telehealth. The role involves leading evidence-based care within the Virtual Hospital Pilot and supporting regional partnerships for heart failure patients.

You will provide expert, patient-centred clinical care, working with the Cardiology team and GPs, and contribute to systems improvement in heart failure services.
