You will work closely with support workers, clients, and their families to ensure safe, effective, and person-centered service delivery that aligns with NDIS quality standards and aged care best practices.

Key responsibilities
for the Registered Nurse include:
* Conducting comprehensive health assessments and developing individualised care plans for participants.




* Providing clinical nursing interventions, medication management, and wound care in home-based settings.
* Monitoring client health outcomes and updating care plans as clinical needs evolve.
* Collaborating and communicating effectively with support workers, allied health professionals, and family members.
* Ensuring all clinical documentation is accurate, up-to-date, and compliant with relevant regulatory frameworks and company policies.
* Providing guidance and mentorship to support staff regarding basic clinical needs and care standards.
To be successful in this position, you must hold current registration with the Nursing and Midwifery Board of Australia (AHPRA) as a Registered Nurse. We are looking for a professional who demonstrates robust communication skills, empathy, and a genuine passion for community-based care. Experience within the aged care or disability support sector, particularly working under the NDIS framework, will be highly regarded.
This is a permanent, full-time opportunity offering a competitive salary range of A$75,000 to A$85,000 per year.
