The Transitional Aged Care Program based at War Memorial Hospital provides time limited, multidisciplinary support and low level rehabilitation and reablement therapy to community clients.
The AIN / AHA will work within a agile multidisciplinary team to provide therapeutic intervention and support to aged clients delivered in their homes following a period of hospital care. We are looking for an enthusiastic and flexible AIN / AHA who values helping older adults maximise their quality of life.
