Hassett Group is seeking a Chronic Disease Management Nurse for a full time role in Port Macquarie, NSW. The position focuses on patient education, care planning and preventative health within a supportive medical centre team.

You will work closely with GPs and allied health professionals to deliver patient-centred chronic disease management and health assessment services, conducting assessments, preparing care plans and coordinating referrals.
