- Transition Care Program Bayside Health is a public health service delivering high-quality care across every stage of life for close to 1.2 million people living in metropolitan Melbourne, the Mornington Peninsula, Koo Wee Rup, Bass Coast and Southern Gippsland. As a Grade 2 Physiotherapist, you will play a key role in delivering high-quality physiotherapy assessment and management across a broad range of aged care clinical streams, helping to optimise patient outcomes and support safe transitions from hospital care. Working within a collaborative multidisciplinary team, you will contribute to person-centred care, goal setting,



rehabilitation and service improvement initiatives while supporting positive outcomes for older people across community and home-based settings throughout the Mornington Peninsula. - Bachelor of Physiotherapy, Masters of Physiotherapy Practice.
- Current general registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- A minimum of 2 years professional experience in working with older adults.
