Disability Support Worker – Community Access

Location: Sunshine Coast, Queensland
Employment Type: Independent Contractor operating under an ABN

We are seeking a reliable, personable and professional Disability Support Worker to provide 1:1 community access support to a young gentleman.

Key Responsibilities:

- Support attendance at appointments.
- Assist with shopping and everyday errands.
- Accompany the participant to cafés, parks, local attractions and other agreed destinations.
- Support participation in recreational activities and personal interests.
- Provide transport for agreed community activities.
- Offer companionship and support positive social engagement.
- Encourage participant choice, independence and confidence.
- Work towards the participant’s identified NDIS goals.
- Follow the participant’s support plan and maintain accurate progress notes.
- Communicate relevant observations or concerns to the coordination team.

Applicants must hold, or obtain before commencing where applicable:

- An active Australian Business Number (ABN).
- Certificate III in Individual Support or an equivalent relevant qualification.
- Current NDIS Worker Screening Check.
- Current National Police Check.
- Current First Aid and CPR certification.
- Completion of the NDIS Worker Orientation Module – Quality, Safety and You.
- A reliable, comprehensively insured vehicle suitable for transporting the participant.
