Avanti Health Centres is seeking a part-time Care Partner to manage a Support at Home caseload, develop care plans and budgets, and coordinate services with our team and partner providers. You will ensure funding is used effectively while maintaining solid client and family relationships.
You will also monitor budgets, conduct regular reviews, and liaise with GPs and community services to optimise care. A reliable vehicle and current checks are expected.
