IPC Health is seeking a Part Time Aged Care Assessor to deliver Home Support Aged Care assessments across Hobsons Bay, Brimbank, Melton and Wyndham. You will conduct holistic assessments for clients 65+ (50+ for Aboriginal or Torres Strait Islander people) using the Integrated Assessment Tool via My Aged Care.
You will develop team-oriented Support Plans to promote independence, empower clients, and ensure data accuracy, quality care and regulatory compliance while contributing to service
